Amanda Wilson: Moving from Abstinence to Harm Reduction in the Age of Physical Distancing

An abstinence-based approach to physical distancing says here is a list of things you shouldn’t do; if you are doing those things you are bad, the only way to be good is to not do those things. A harm-reduction approach to physical distancing asks what are the things that are important to us, and how can we conceive of doing those things in ways that minimizes the harm and risk.

The idea that we should abstain from a long list of activities needs to evolve to a set of guidelines, practices, and adaptations that enable us to live together while reducing the potential for risk and harm.  This doesn’t mean we’ll be able to do all the things we used to, but it acknowledges that people have different needs and different circumstances and we need to find a way to work with those.

Only interacting with people in your household is easy (or perhaps easier) if you live with someone else (whom you like spending time, and who is healthy and safe for you to be interacting with), and if you live in a situation that has enough space for those interactions to be comfortable. If you live alone, if you live in a cramped or unsafe household, that becomes much more difficult, and potentially much unhealthier. Similarly, interacting online with friends and family is easy if you have access to the right technology, a strong internet connection, and if the people you are seeking to connect with also have those things. All of us need to do different things to maintain our mental and physical health – exercise, meditation, eating particular foods, accessing certain services and support.  Our ability to do these things under the current protocols varies greatly, and we need to think of ways of living, collectively, that enable all of us to do those things.

The fact is that BOTH the government and the population have huge reasons not to trust each other.

"Providing top-down guidance with such obvious contradictions backfires exactly because lack of trust is what fuels hoarding and misinformation," she said, adding that if officials were concerned about shortages, they should have stated this outright, and asked people to donate masks they had bought to hospitals, not claimed they were ineffective in the first place. 

"When people feel as though they may not be getting the full truth from the authorities, snake-oil sellers and price gougers have an easier time."

Trying to avoid mask shortages for healthcare workers appears to have been the main priority of those arguing against widespread their widespread use. But while the motivation behind this may have been good, such advice may have actually helped spread the virus, thus adding to the number of patients overwhelming hospitals. 

One of the reasons the CDC's Redfield gave for potentially changing the guidance on masks is that the coronavirus can be spread when people are asymptomatic, and therefore having everyone covering their faces -- as has been the norm in Hong Kong and other parts of Asia since January -- could help rein in transmission. 

That the virus is spread before people feel sick is not news, however. It's been widely supposed since the first weeks of the outbreak, and evidence has only grown stronger in recent months. 

And even if there was not asymptomatic transmission, universal or near universal mask wearing has its uses. As others have noted, instructing only the sick to wear masks is essentially asking people to put a sign on themselves inviting fear and hostility, whereas if everyone wears a mask when outside, the sick are more likely to do so, thus protecting people around them. 

The lack of masks and other protective equipment in hospitals across the US and other countries should be remedied as fast as possible. But these shortages were caused by policy failures and supply chain issues, not a sudden run on 75 cent papers masks by concerned members of the public. 

As the evidence increasingly comes round in favor of masks, we must ask how many infections might have been avoided if in January, instead of saying masks wouldn't help, officials and the media had instead lobbied for factories to ramp up domestic production, provided guidance on how to make masks at home, and asked other countries to donate surplus materials?

I think the problem is a social one. The people who need the masks are the ones with the virus.

Yes some will have it before knowing so there is value there.

The problem with recomendation regarding masks is that this makes no sense. and here is why:

The people who will follow this wil be the ones most responsible and careful - disproportionately not those who are less liekly to disregard social distancing and get it.

I see a logic in recommendng that those who are not sick do not wear a mask until such time as you say everybody must wear a mask. In that case you also have to guarantee a supply. To make matters worse, you ahve to manage people wasting them and hoarding them.

In other words the difficulty in these instructions is that people are arseholes.

All that said I thin governments should ahve moved very quickly to check stockpiles of masks when it was in China (they did not do this) and then move to make them rapidly (again they did not do this) and then buy up supply controlling it. With this done the government could have rationed supply to everyone and told them that they HAD to use them to be allowed in grocery stores or any open public building.

Governments have been doubling down becuase the screw-ups started in some cases years ago maintaining supply and have been compounded through this. Each time the govenrment makes decisions so slow that the measures become out of date and more ineffective than if they ahd been done from the start. Some of this is inexcusable slow reaction and some is a tendency to wait to confirm justification instead of working out of safety proactively. 

It seems that this general mistake has happened in almost every other country. Some countries latched on to a single response and then did not escalate. They closed borders to buy them time and instead wasted it in the hope that this measure could escape it.

The wealthier world did not adequately fund and prepare themselves. This means they are in a crisis at a time when they should be funding and helping poorer countries.

Many countries have been wasting time designing policies with various groups in mind leaving people fallng through the cracks (Not only am I one but my partner as well for a different reason). So we are two people with zero income in the same home. A universal benefit would have been easier faster and comperehensive. There is no excuse for this as people could simply have been warned that next year's taxation would even this out. Special tax measures could ahve looked at issues like what people were making before, what their cash flow is during these months, what income they were able to earn during this period. This would have been much easier. They could have simply issues this money and produced T4 for the year up to the emergency; T4 for the period of the emergency and then another (if we get out in this year) after. They could have taxed each seperately. This would have clawed back assitance from those who still had an income during the period while letting people who had a higher income before still get help as they were caught by suprice. This system that sounds alittle complicated would have been easier than anything else. It would have allowed money to roll out now and the accounting to come out later. It would have provided a basic income for everyone taking back from those who did not need it at the ned of the year.

Response has been dreadful.

I cannot find the link but will share this asking those to beleive me as I have never lied about something factual here.

New research is questionning the notion that 6 feet distance is enough. they found droplets went as far as 29 feet. this means if someone without symptoms in a store sneezes less than 30 feet away, you could get it.

Do not shop in stores without plexiglass shields for staff. Do not shop where stores are not controling numbers in the store strictly. Avoid Walmart (they are not doing enough and their stores are crowded). A better alternative seems to be Food basics (Sobey's) as they are both using plexiglass shields and controlling those entering. 

Freshco and others are santizing at night and giving seniors the first hour in  a cleaned store. this is only good so long as the seniors are not too numerous.

Research shows that not only are seniors more liable to become critically sick but they are also more likely to be infected. This has to do with reduced ability to kill off tiny amounts of infection. Seriously if you are over 65 -- strongly consider staying in isolation and getting people to bring you items you need. Do not go out.

If you are on this site and a senior and you need anything and you are in Ottawa. Message me and I will get it for you and hang it on your door knob without contact with you (I can phone once there to let you know it is there. Seriosuly if you are older do not go out. This is life or death these next few weeks.

Since unlike South Korea and Taiwan and Japan and China we do not live in an Advanced Modern Economy we do not have the capability to respond to this crisis. We are exporters of raw materials and do not have the  robust capability of AME Asian economies to ramp up production in existing plants. Hell even their cellphone apps out perform the shit on offer from our Old Order, billionaire owned, cyberspace.  Our politicians response from BC and Alberta and Ottawa is to throw more resources at building carbon export infrastructure. Fuck the millions being spent on TMX everyday, shut it down and offer incentives to start up things like mask manufacturing in every region of the country. Canadians would gladly start wearing masks if they could go to a store and buy a dozen for a reasonable price. But then we aren't an AME we are Old Order.

The COVID-19 pandemic has affected at least 590,000 people and claimed tens of thousands of lives — and yet politicians and fossil fuel CEOs are continuing to put thousands of workers, and people in the northern and rural communities they are working in, at severe risk for oil and gas profits.

I have moved into the support everyone wearing a mask camp, once the health care workers have ample supplies, and they are made available to the general public, when I leave the house. I am quite concerned about the trust issue and our leaders need to tell us the truth. Having said that I don't appreciate the grandstanding by some of our BC doctors, who are undermining Dr Bonnie. Now my question is, when will BC get enough masks for all its citizens?

Portugal gives migrants and asylum-seekers full citizenship rights during coronavirus outbreak

Portugal has temporarily given all migrants and asylum seekers full citizenship rights, granting them full access to the country's healthcare as the outbreak of the novel coronavirus escalates in the country.

The move will "unequivocally guarantee the rights of all the foreign citizens" with applications pending with Portuguese immigration, meaning they are "in a situation of regular permanence in National Territory," until June 30, the Portuguese Council of Ministers said on Friday.

The Portuguese Council of Ministers explained that the decision was taken to "reduce the risks for public health" of maintaining the current scheduling of appointments at the immigration office, for both the border agents and the migrants and asylum seekers.....

Here is a good news story. I love my Village because we are a real community. Here are how my neighbours are stepping up to ensure no one is left behind.

n partnership, Cumberland Community Schools Society (CCSS) and Weird Church Cumberland have created Cumberland Food Share: a two part way to provide support for individuals and families in Cumberland facing food insecurity. With support from the Village of Cumberland, Lush Valley Food Action Society and the Comox Valley Food Policy Council, this new program will distribute food safely to Cumberland families and individuals in need of support during COVID-19 (and hopefully beyond!).

This is a two part program:

The Food Shed: Thanks to Cumberland Crate Company, Courtenay Glass Ltd, Gareth Adams, and Vig Schulman there is now (almost!) an outdoor, non-perishable food pantry available 24/7 at 2688 Penrith Ave! It is the hope and prayer of Weird Church that our neighbours who have some food to spare will add to it, and that neighbours who are in need might take what they need.

Good Food Boxes: Weekly hampers for community members who are experiencing challenges due to loss of income and inability to access food, especially those who have lost access to the Breakfast and Lunch Programs available through CCSS. To address this community need, Cumberland Food Share will purchase, sort and distribute boxes which will include food, gift cards, and supplies to those who have expressed need.

Two top administration officials last year listed the threat of a pandemic as an issue that greatly worried them, undercutting President Donald Trump's repeated claims that the coronavirus pandemic was an unforeseen problem.

Health and Human Services Secretary Alex Azar and Tim Morrison, then a special assistant to the President and senior director for weapons of mass destruction and biodefense on the National Security Council, made the comments at the BioDefense Summit in April 2019.

"Of course, the thing that people ask: 'What keeps you most up at night in the biodefense world?' Pandemic flu, of course. I think everyone in this room probably shares that concern," Azar said, before listing off efforts to mitigate the impact of flu outbreaks.

The Trump administration is facing scrutiny over its preparations for the coronavirus pandemic and its slow response to provide states and cities assistance in testing kits and personal protective equipment. The 2019 summit, hosted by the assistant secretary for preparedness and response in the Department of Health and Human Services to "discuss and solicit input on implementing the National Biodefense Strategy," offers insights into early awareness of the potential for a pandemic threat.

When this virus first started up I had already made plans for an international trip but had some concerns about traveling on a plane with potential virus carriers, and discussed it with a close friend whose opinions I value greatly, and he said if it were he traveling, he would wear a mask. And of course I was seeing nightly on the TV News that people in Hong Kong / China were wearing masks. But the WHO, the Canadian Government, etc. were all saying not to wear masks so understandably I had some confusion. These racist attacks on Chinese people over the virus however, in an attempt to discredit them, are a threat to the world community, as the Chinese approach was correct.
